Multiple Teams · Reminders for Fall Sports Student-Athletes/Parents

Thanks to all who attended Monday night’s Fall Sports Meeting. Below is a recap of things we discussed and some links that may be helpful!

FinalForms must be updated and signed by both the parent and the student-athlete BEFORE the first practice or try-out (incoming freshmen will be able to sign their student forms at orientation on August 14th). The link to log in is here. New users will need to create an account and past users will need to verify information and re-submit electronic signatures for this school year. For help with FinalForms, please visit the FinalForms Playbook by clicking here.

Please make sure to check the sports your daughter is planning on playing in the 2019-20 school year!

Physical Examinations

According to the Ohio High School Athletic Association’s bylaws (Section 4, Bylaw 3-4-1): “Athletic participation forms for participants in any of the grades 7-12 shall be signed by a medical examiner (Physician [M.D., D.O., or D.C.], Advanced Nurse Practitioner or Physician’s Assistant), the participant and by a parent or guardian. These forms must be on file with the principal or designee (athletic office at Beaumont School) before any candidate for a team may participate in a practice. These forms shall require the medical examiner to certify the individual’s physical fitness no less than once each calendar year.

OHSAA’s 2019-20 Pre-Participation Physical Evaluation Form can be found here.

Team Fees

Beaumont School charges a team fee to participate in athletics. This fee is used to help defray the costs of transportation, officials, equipment, uniforms and entry fees. If a student chooses to quit the team, the fee will not be refunded.

Team fees are due by Friday, August 30th.

You can pay online here or make a check payable to Beaumont School. If writing a check, please be sure to include the sport in the memo and drop it off in the athletic office or mail to Beaumont School, Attn: Athletics.

The following fee schedule will be in effect for the 2019-20 school year:

FALL – Due by Aug. 30, 2019
Cross Country $280
Golf $320
Soccer $280
Tennis $320
Volleyball $320

Season Tickets / North Coast League Pre-Sale Tickets

Tickets for JV/varsity soccer and volleyball matches are $7 for adults and $5 for students in K-12 and senior citizens. Tickets for freshmen volleyball matches are typically $3 for adults and $2 for students and senior citizens.

Beaumont sells season ticket punch cards which can be purchased by filling out this form. We also sell pre-sale tickets for North Coast League road games and the form to purchase those discounted tickets is here. By purchasing tickets for NCL road games, the revenue stays with Beaumont rather than the host site.
