Other info is below regarding physical exams, FinalForms and team fees.
According to the Ohio High School Athletic Association’s bylaws (Section 4, Bylaw 3-4-1): “Athletic participation forms for participants in any of the grades 7-12 shall be signed by a medical examiner (Physician [M.D., D.O., or D.C.], Advanced Nurse Practitioner or Physician’s Assistant), the participant and by a parent or guardian. These forms must be on file with the principal or designee (athletic office at Beaumont School) before any candidate for a team may participate in a practice. These forms shall require the medical examiner to certify the individual’s physical fitness no less than once each calendar year.
OHSAA’s 2017-18 Pre-Participation Physical Evaluation Form can be found here.
A student-athlete can’t practice on February 19th without a physical exam.
FinalForms must be updated and signed by both the parent and the student-athlete BEFORE the first practice or try-out. The link to log in is here. New users will need to create an account and past users will need to verify information and re-submit electronic signatures for this school year. A “playbook” with FAQs about FinalForms can be found on BeaumontAthletics.org under the ‘Forms’ tab.
Please make sure to check the sports your daughter is planning on playing in the 2017-18 school year!
