Below is important information regarding physical exams, FinalForms and team fees for the spring season at Beaumont.
Lacrosse and softball practice can start on Monday, Feb. 19 while outdoor track and field can start on Monday, March 5.

According to the Ohio High School Athletic Association’s bylaws (Section 4, Bylaw 3-4-1): “Athletic participation forms for participants in any of the grades 7-12 shall be signed by a medical examiner (Physician [M.D., D.O., or D.C.], Advanced Nurse Practitioner or Physician’s Assistant), the participant and by a parent or guardian. These forms must be on file with the principal or designee (athletic office at Beaumont School) before any candidate for a team may participate in a practice. These forms shall require the medical examiner to certify the individual’s physical fitness no less than once each calendar year.

A student-athlete can’t practice on February 19th without a physical exam.
FinalForms must be updated and signed by both the parent and the student-athlete BEFORE the first practice or try-out. The link to log in is here. New users will need to create an account and past users will need to verify information and re-submit electronic signatures for this school year.
